- Download the requisition form from the HSTU Central Laboratory website or Central lab/Office, HSTU. Download Requisition Form
- Pay the appropriate service charge to Account number 218 (Rupali Bank, HSTU).
- Drop the form and payment slip at the Central lab/Office.
